I'm not sure if it'll be of any use to you at all, but "How to cultivate a daily meditation" (I think that is the title) written by His Holiness the Dalai Lama contains some instructions on how to set up an alter. However I would also agree with Stephen in the respect that it should be personal and perhaps contacting your Lama (if you have one) or visiting a temple would be a good way to get further information. Hope it all comes together for you.
